- This form should be used when a New Unit is applying for submission.
- This form should be returned to SA IMET no later than 60 days prior to the date requested for the Survey Visit.
- When applying for accreditation, facilities should allow sufficient time for required approval processes.

Facility Accreditation Submission
- This form should be completed when a Facility is applying for accreditation.
- This form should be returned to SA IMET no later than 60 days prior to date of the Survey Visit. If you need to provide additional information please attach a separate sheet to the submission.
- Details on the submission are to be verified by the Director of Medical Services and the Director of Clinical Training prior to submitting to SA IMET.

Term Description Template
This document is designed to provide important information to junior doctors regarding a particular rotation. It is best regarded as a clinical job description and should contain information regarding the:
- Casemix and work load
- Roles & Responsibilities
- Supervision Arrangements
- Contact Details
- Weekly timetable
- Learning Objectives
The Term Description may be supplements by additional information such as Clinical Protocols which are term specific. Term Supervisors should have considerable input into the content of the Term Description and they are responsible for approving the content.
In determining learning objectives, Supervisors should refer to the Australian Curriculum Framework for Junior Doctors (ACFJD).
The Term Description is a crucial component of Orientation to the Term however it should also be referred to during the Mid Term Appraisal and End of Term Assessment process with the junior doctor.
*2011 Accreditation Timeline: New / Modified Units*
If a hospital believes that they are likely to require a new unit or request modifications to an existing unit to be ready for 2012, they are asked to advise SA IMET by the 31st May 2011.
It is recommended hospitals do not wait for final funding approval, as this could result in the unit not receiving accreditation status prior to the require commencement date.
In the case where accreditation is granted but the funding is not approved, the accreditation status will remain on file for an appropriate period of time pending funding approval. Once funding has been granted the hospital will need to advise SA IMET of any substantial changes that would affect the original accreditation decision, as per the Notification of Change of Circumstance document
